DECLARATIONS: This Agreement is entered into between personal trainer Calee Shea Lunsford (“Trainer”) and the undersigned (“Client”). The provision of personal training services by Trainer to Client, and Client’s use of any premises, facilities or equipment are contingent upon this Agreement.
