This Collaborative Practice Agreement for Rapid Antigen Strep Testing is between Dr. XXX and the pharmacists employed at Pharmacy XXXXXX (NCPDP # XX-XXXXX) at 906 South Monroe, Spokane, WA 99204. These pharmacists have reviewed the current practice guidelines for GABHS and/or completed the credentialing program titled, “A Primer on Community Pharmacy-based Rapid Antigen Testing for Strep Throat,” which includes the current guidelines and recommendations of the Infectious Diseases Society of America. Other strep testing information resources, such as the Washington State Department of Health and CDC, are available for the pharmacists to review. In addition, Dr. Garman will provide in-service presentations or allow pharmacists to participate in a clinical rotation at the clinic in order to illustrate proper strep throat evaluation techniques.
