This is an agreement between ………………………………………………………………..(parent/s), parent of ……………………………… and Denise McDermott (therapist) prior to the commencement of child psychotherapy therapy sessions. Please note both parents must sign this form where both parents are recognised as legal guardians of the child. If this is not possible to obtain, I will discuss this with my child’s therapist.
