entered into this 18th day of December 2008 ending June 30, 2009, between the School Board of Clay County, Florida (hereinafter referred to as “the BOARD”) duly operating under the Laws of the State of Florida, 900 Walnut Street, Green Cove Springs, Florida 32043 and Sylvan Learn Center, 1414 Kingsley Avenue, Orange Park, Florida 32073, Supplemental Educational Service Provider (hereinafter referred to as “the PROVIDER”) for the purpose of providing Supplemental Educational Services (SES) to eligible students as agreed upon in the Student Learning Plan (SLP). Eligible students are those students that have been identified by employees of the BOARD who meet specific requirements under Title 10, Part C of the No Child left Behind Act of 2001.
