THIS PERSONALIZED CARE MEMBERSHIP AGREEMENT (this “Agreement”) is made effective as of , by and between the undersigned member and, if applicable, additional members listed on Schedule 1 hereto (each, a “Program Member”), and Coral Springs Personalized Care, a Florida limited liability company, having an address of 3100 Coral Hills Dr., Suite 303, Coral Springs, FL 33065 (“Personalized Care Practice”; and together with Program Member(s), the “Parties”).
