This Agreement by and between Cosmo Wireless LLC a Limited Liability Company having its principal place in Caruthersville, Missouri and the customer whom name appears in Appendix A/ Sales Order (hereinafter Customer) is effective as of the effective date shown in Appendix A. Cosmo Wireless LLC agrees to provide wireless internet service to the Customer at the prices and for the term set forth in Appendix A. Customer agrees to abide by the terms and conditions of this Agreement and any appendices attached hereto and to pay Cosmo Wireless LLC the complete price for the service.
