This Medical Services Agreement (the "Agreement") is made and entered into on January 30, 2013 (effective from July 1, 2012 to June 30, 2013), by and between the City of Burbank (the "CITY"), and Providence Health System - Southern California d/b/a Providence Saint Joseph Medical Center ("PSJMC"), and the Burbank Emergency Medical Group (“BEMG”) (hereafter may be referred collectively to as the “Parties”) for the purpose of the CITY using the services of PSJMC and BEMG to perform certain medical tests, medical evaluations and limited treatments (“collectively referred to as “Services”) to certain persons who (i) are arrested or taken into custody by CITY (“Arrestees”), or (ii) are booked and jailed in CITY facilities (“Prisoners”).
