, (Provider) enters into a Provider Agreement with the State of Alaska, Department of Health & Social Services (DHSS) for the purpose of providing heating system replacement services to Alaskan households with gross monthly income below 150% of the federal poverty income guidelines whose heating equipment is inoperable or unsafe for the State of Alaska’s LIHEAP Program. By entering into this Provider Agreement, the Provider agrees to the following, including all applicable provisions of Appendices A,C,D and E and Attachments 1 and 2.
