Medical Associates Health Plan, Inc. is licensed to do business as a health maintenance organization in the states of Iowa, Illinois, and Nebraska and Medical Associates Clinic Health Plan of Wisconsin, is licensed to do business as a health maintenance organization in the state of Wisconsin.
