This Administrative Services Agreement (“Agreement”), entered into the day of _, 20 , by and between PNC Bank, National Association (“PNC,” “we,” “our,” or “us”) and the Employer. As used in this Agreement, the term “you”, “your” and “yours” refer collectively to the Employer. This Agreement contains the terms and conditions under which we will provide certain Services to you. These General Terms and Conditions apply to all of the Services described in this Agreement. The Terms and Conditions for each Service is described in the applicable HSA schedule (“HSA Services Schedule”). We may provide additional implementation documents which, if applicable to a Service, we will supply to you before that Service is provided. The Agreement consists of these General Terms and Conditions along with any other documents, including but not limited to the HSA Services Schedule, that has been agreed to by the parties and that is specifically incorporated into this document by reference.
