(the “Participant”) and P hy s i c i a n s A ccountable C are S o l u t i o n s , L L C ( t he “ ACO” ), effective the 30th day of July, 2015 (“Effective Date”), allows any physician or other licensed health care provider or supplier who is employed by or is contracted with the Participant and who bills for items and services furnished to Medicare fee-for-service beneficiaries under the Medicare billing number assigned to the tax identification number of the Participant (each, a “Participating Provider”) to participate in the ACO’s network of providers (“Network”) with respect to the agreement ACO has or intends to enter into (“MSSP Agreement”) with the Centers for Medicare & Medicaid Services (“CMS”) for the Medicare Shared Savings Program (“MSSP”), with the participation in the ACO by the “Participant” to be effective as of January 1, 2016.
