Contract Type FiledFebruary 2nd, 2021Criteria for shared care Prescribing responsibility will only be transferred when:• Treatment is for a specified indication and duration (if applicable)• Treatment has been initiated and established by the secondary care specialist• The patient’s initial reaction to and progress on the drug is satisfactory• The patient’s general physical, mental and social circumstances are such that he/she would benefit from shared care arrangements Patients will be excluded from shared care if the following apply:• Unstable disease state• Patient does not meet criteria for shared care Patients should be referred back to the specialist as per the agreed treatment plan in the following situations:• The clinical situation deteriorates such that the shared care criterion of stability is not achieved• The clinical situation requires a major change in therapy LMWHs are considered to be safer than warfarin and other oral anticoagulants for the treatment and prophylaxis of venous thromboembolism (VTE) in cer
