This Data Use Agreement is dated , , and is between the (“Recipient”) and University of Miami, (“Covered Entity”). This Data Use Agreement is made in accordance with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), the Health Information Technology for Economic and Clinical Health (“HITECH”) Act, and the final omnibus rule published in January 2013.
