This Maryland Participating Pharmacy Agreement (“Agreement”) is made by and between ProCare Pharmacy Benefit Manager, Inc. (herein “ProCare”), organized under the laws of the State of Florida, with its principal place of business located at 2650 SW 145th Avenue, Miramar, Florida 33027, and the undersigned Pharmacy (herein “Pharmacy”), collectively referred to as the “Parties”, to be effective (“Effective Date”), for the purpose of setting forth terms and conditions under which Pharmacy Services shall be rendered.
