This MASTER REGISTRY AGREEMENT (“Master Agreement”) is made and entered into as of the “Effective Date” identified below, by and between (i) the American Board of Family Medicine, Inc. (“ABFM”), a Missouri non stock, non-profit corporation located at 1648 McGrathiana Parkway, Suite 550, Lexington, Kentucky 40511, (ii) the individual or organization registering for the “Registry” (identified below as a “Participant”), and (iii) each individual physician (“Physician”) for whom the Participant is registering as part of the Participant’s participation in the Registry. The ABFM, Participant and the Physician are referred to collectively as the “Parties”).
