THIS PERSONALIZED CARE MEMBERSHIP AGREEMENT (this “Agreement”) is made effective as of the Effective Date, , by and between the undersigned member and, if applicable, additional members listed on Schedule 1 hereto (each, a “Program Member”), and POTOMAC INTERNISTS, P.C., a District of Columbia corporation, having an address of 5215 Loughboro Rd. NW, Suite 400, Washington, DC 20016. (“Personalized Care Practice”; and together with Program Member(s), the “Parties”).
