The Nasdaq Stock Market LLC (“Nasdaq”), as Administrator of the UTP Plan (“UTP Plan Administrator”), requires all Subscribers to the information described herein (“Information”) to sign the UTP Plan Subscriber Agreement (“Agreement”), or its equivalent, in order to receive the Information. (Refer to definition in Section 153). By completing the below section, the Subscriber agrees to the terms and conditions set forth in this UTP Plan Subscriber Agreement.
