This Exhibit [A] Business Associate Agreement (“BAA”) is attached and expressly made part of the Customer Agreement (“Agreement”) between DoctorLogic, LLC (“DL”) and the Customer referenced in the Agreement. The Agreement is inclusive of DL’s standard Terms & Conditions (“T&C”) and any other Exhibits identified in or attached to the Agreement. For purposes of this BAA and the Agreement, “Business Associate” means DL and “Covered Entity” means Customer referenced in the Agreement. This BAA is expressly subject to and incorporates by reference the T&C, including the provisions of the T&C with respect to indemnification and limitation of liability.
