UC Health, LLC, an Ohio non-profit limited liability company, on behalf of University of Cincinnati Medical Center, LLC (“UCMC”), an Ohio non-profit limited liability company, and Resident (as herein defined) have executed a Graduate Dental Education Contract (the “GDE Contract”) which incorporates these 2019/2020 University of Cincinnati Medical Center Graduate Dental Education Standard Terms & Conditions and the attached Exhibits A and B (collectively, the “Standard Terms & Conditions”) in their entirety. Throughout these Standard Terms & Conditions the term “Residency Program” shall refer to the course of graduate dental education in which Resident trains for practice in general dentistry in accordance with the standards of the American Dental Association (ADA) Commission on Dental Accreditation (CODA).
