This Business Associate Agreement, dated as of January 18, 2023 (the “Agreement”), is entered into between Oregon Health & Science University, through its Oregon Rural Practice- based Research Network (the “OHSU”) and Lincoln Community Health Center, on behalf of itself and its subsidiaries and affiliates (the “Lincoln Community Health Center”), each a “Party” and collectively, the “Parties.”
