This End User License Agreement (“Agreement”) is incorporated by reference into the Program Agreement and DAS Standard Incorporated Terms and Conditions as of the date first executed (“Effective Date”) by and between Henry Schein Medical Systems, Inc., an Ohio corporation (“HSMS, INC.”), DAS Health Ventures, Inc. (“DAS”) and Client (as defined in the Program Agreement).
