This Contract will become effective March 1, 2017, and is by and between the Texas Juvenile Justice Department, hereinafter TJJD or Department, and Deer Oaks EAP Services, LLC, 126 E. Main Plaza, San Antonio, Texas 78205, hereinafter Service Provider or Contractor, for the purpose of providing an Employee Assistance Program (EAP) for TJJD. This Contract expires on August 31, 2018.
