This CalAim Community Supports Provider Participation Agreement (“Agreement”) is made and entered into by and between the Community Supports (CS) Provider identified on the signature page of this Agreement (“Provider”), and Health Net of California, Inc. on behalf of itself and its subsidiaries and affiliates (collectively, “Health Net”).
