In consideration for permitting the above named player to participate in full contact football with the St. Charles Football League (STCFL), I the undersigned parent and/or legal guardian of the above named player do hereby consent and agree that the above named player may participate with the STCFL. It is agreed that STCFL sponsors and coaches assume no legal liability for injuries or other loss as a result of such participation.
