This agreement, made up of these Master Terms and Conditions (“Master Terms”) and any and all Orders (as defined below) and any applicable specific terms and conditions (as defined below) (collectively, the “Agreement”), is agreed between the customer identified on an applicable order form (“Customer” or “you” or “your”) and Carestream Dental LTD (together with its affiliates, “Carestream Dental”, “CSD”, “we”, “us”, or “our”), a United Kingdom limited liability company with an office located at Wiltron House,Rutherford Close, Stevenage, Hertfordshire, SG1 2EF (each a “Party”, collectively, the “Parties”), intending to be bound.
