This document model affiliation agreement has been reviewed and approved by the CACMS as meeting the requirements of CACMS Standard 1, Element 1.4 Affiliation Agreements. It is meant to serve as a guide for Canadian medical schools to ensure that the agreements between the medical school and its clinical affiliates address the areas specified in the accreditation element.
