Current Vaccination/Veterinarian Information: By signing this contract, owners verify their pets are current on Rabies, Distemper, and Parvo-Virus and Bordatella (kennel cough). Proof of Vaccination or current Titer testing shall be provided to xxx LLC upon request as well as current Veterinarian information. Special circumstances will be taken into consideration. Also know and understand that even a fully vaccinated pet can still contract illness (kennel cough, parvo, etc.) and you will hold harmless xxx and members if your pet does become ill. We maintain a clean and sanitary environment and will not allow any KNOWN sick pets into our establishment for the safety of the other pets. _________ (client initial)
