THIS CLINICAL REGISTRY AGREEMENT (“Agreement”) is made and entered into as of the 27 day of March 2017 (hereinafter “Effective Date”) by and between BioMarin International Limited, an affiliate of BioMarin Pharmaceutical Inc., incorporated under the laws of Ireland, whose principal place of business is at Shanbally, Ringaskiddy, County Cork, Ireland (hereinafter “Sponsor” or “BioMarin”), and General University Hospital in Prague, located at U Nemocnice 499/2, 128 08, Prague 2, Czech Republic, Company ID No. 00064165 (hereinafter “Institution”) (each herein referred to as a “Party” and collectively as “Parties”).
