This purchase agreement (this “Agreement”) is entered into by and between (“Supplier”) and CVS Pharmacy, Inc., a Rhode Island corporation, on its own behalf and on behalf of its relevant store subsidiaries and affiliates (“Retailer”), and will be effective until terminated by either party upon thirty (30) days’ written notice to the other party. Park City Group Inc. is the “Designated SBT Processor” as of the effective date of this agreement.
