The Clinic is a Direct Pay primary care practice (DPC) which delivers primary care services through its physician, Dr. Theresa C. (Dr. Tess) Garcia, at 1416 NW 7 Highway, Blue Springs, Missouri 64014. In exchange for certain fees, the Clinic agrees to provide the Patient with the Services described in this Agreement on the terms and conditions contained in this Agreement.
