EMPLOYEE BENEFITS TRUST (“Trust Agreement” or “Agreement”) is made and entered into this day of , 201_, between Mesa Unified School District No. 4, acting by and through its Governing Board (hereinafter referred to as “District”), and certain persons (hereinafter referred to collectively as the “Trustees” and individually as a “Trustee”) for the administration of the Trust known as the “Mesa Public Schools Employee Benefits Trust” (hereinafter referred to as the “Trust”), in accordance with the terms and provisions of this instrument.
