WHEREAS, USA is a Preferred Provider Organization (PPO) engaged in the business of administrating quality health care services at an affordable price through its products {USA H & W Network, a group health and wellness program, USA Workers’ Injury Network (USA WIN), USA Medicare Select, HMO} and Facility desires to provide services for the members (hereinafter referred to as “INSUREDS”) of various group accident/health plans, work related injury/illness plans, motorist medical plans, Medicare Select plans, Health Maintenance Organization plans, and self-insured employers, which have entered into agreements with USA, (hereinafter referred to collectively as "INSURERS"); and
