This agreement is entered into by the Iowa Department of Health and Human Services (hereinafter referred to as the Department) and the individual provider or the group practice or clinic listed in section XIV of this agreement (hereinafter referred to as the patient manager or PM). This agreement outlines the duties and responsibilities of the parties and shall continue in force until amended by the Department pursuant to section XII or terminated by either party pursuant to section XIII. Renegotiation is not allowed.
