This Clinical Trial Agreement (“Agreement”) is entered into as of the date the Cellworks Account is created (“Effective Date”) by and between the Physician (“Physician”) and Cell Works Group Inc. (“Sponsor”), a Delware corporation with its offices at 400 Oyster Point Blvd. Suite 207, South San Francisco, CA 94080 (individually, each a “Party”; collectively, the “Parties”).
