This agreement is entered into the 1st day of July, 2019 between Prisma Health (previously known as Palmetto Health) or “Sponsoring Institution,” a multiple teaching hospital/health system, located in Columbia, South Carolina (hereinafter “Prisma Health-Midlands”) and XX, MD/DO/DDS/DMD (hereinafter “Resident/Fellow”).
