My signature below indicates my acknowledgement that I may only receive actual and necessary expenses related to my rehabilitation during the summer in accordance with NCAA Bylaw 16.4 below. I understand that room and board costs will be provided to me based on average per diem amounts. If I complete rehabilitation earlier than expected, I agree to immediately return any money received in excess of the actual expenses to San Jose State University Athletics. I understand that my failure to return any excess monies will result in an NCAA violation and could result in loss of eligibility.
