This Consolidated Agreement is made between the North Carolina Department of Health and Human Services, Division of Child and Family Well-Being (hereinafter referred to as “DCFW”) and Division of Public Health (hereinafter referred to as “DPH”), (herein DCFW and DPH collectively referred to as “NCDHHS”), and the LHD-Legal-Name (herein after referred to as “LHD”) (herein NCDCFW, NCDPH, and LHD may individually be referred to as a “party” and collectively as the “parties”) for the purposes of maintaining and promoting the advancement of public health in North Carolina. This Consolidated Agreement shall cover a period from June 1, 2023 to May 31, 2024 and shall remain in force until the next Fiscal Year Consolidated Agreement is signed except as provided for in Section X.
