This HIPAA Business Associate Agreement, (“Agreement”) is entered into on the date(s) set forth below by and between Hospital Sisters Health System on its own behalf and on behalf of all its affiliated hospitals and entities, the “Covered Entity”) and (the “Business Associate”) (each a “Party” and collectively the “Parties”). This Agreement shall be effective as of the date on which the last of the Parties has executed this Agreement (“Effective Date”).
