This Data Sharing Agreement (“Agreement”) is made and entered into as of (the “Effective Date”) by and between the (“NAME of TRIBE”) and the Albuquerque Area Indian Health Board, Inc. (AAIHB), the legal contractor for the Albuquerque Area Tribal Epidemiology Center (AASTEC), for purposes of sharing health data stored in the AASTEC managed Southwest Indigenous Database.
