THIS PARTICIPATION AGREEMENT (this “Agreement”) is effective on the later of the execution dates on the signature page below (the “Effective Date”), by and between Community Care Physician Network, LLC (“CCPN”), a North Carolina limited liability company, and the individual or entity identified on the signature page hereof (“Practice”), on behalf of itself and any of its Providers (as hereinafter defined). CCPN and Practice are the “Parties,” and each is a “Party.”
