The Service Organization and the Treatment Provider hereby enter into a QSOA whereby the Service Organization agrees to provide the services outlined in the RHIO Services Agreement between such Parties, dated ( ). By virtue of the RHIO Services Agreement, the Treatment Provider may electronically share certain information with the Service Organization. This QSOA is established in order to satisfy the requirements of 42 C.F.R. § 2.11.
