This Telecommunications Facilities Easement Agreement ("Agreement") is entered into this ____ day of December, 2020, by and between FHMS Four Town Broadband Network (owner of the poles) on behalf of itself and its affiliates ("Partners") including WiValley-MA, Inc, (network operator and owner of electronics) and Town of Heath ("Owner"). Owner holds title to, or is the authorized agent of the titleholder of, certain real property located at _____________ Road, Heath, MA, (“Property”). Owner and Partners desire to enter into this Agreement for Partners to provide its services to the FHMS Four Town Broadband Network subscribers under the terms and conditions herein.
