This Mass Vaccination Site Contract - Amendment and Restatement (the “Amended and Restated Contract”) is by and between the Massachusetts Department of Public Health (“DPH” or “the Department”) and CIC Health, LLC (“Contractor”). The Department and Contractor previously executed a COVID-19 Mass Vaccination Site Contract, effective January 11, 2021 (the “Original Contract”). The parties intend to clarify the contract documents which govern the relationship between the Department and Contractor. This Amended and Restated Contract replaces and supersedes the Original Contract and shall be effective on March 8, 2021.
