This TCView Services Agreement (“Agreement”) is entered into on February 20, 2012 (“Effective Date”) between AT&T Services, Inc., a Delaware corporation, with offices at 175 E. Houston Street, San Antonio, Texas 78205, as authorized agent for Illinois Bell Telephone Company d/b/a AT&T Illinois, Indiana Bell Telephone Company Incorporated d/b/a AT&T Indiana, Michigan Bell Telephone Company d/b/a AT&T Michigan, The Ohio Bell Telephone Company d/b/a AT&T Ohio and/or Wisconsin Bell, Inc. d/b/a AT&T Wisconsin (collectively: “AT&T MIDWEST REGION 5-STATE”) and Home Telephone Co., an Illinois Corporation, with offices at 501 N. Douglas, St Jacob, Illinois, 62281 (“Customer”).
