This Financial Planning/Consultation Client Services Agreement (“Agreement”) is entered into and between Madison Avenue Securities, LLC (“MAS” or the “Adviser”), a Delaware limited liability company that is registered as a broker‐dealer and a registered investment adviser with the Securities and Exchange Commission (the “SEC”), and (“Client”). Adviser has designated the investment adviser representative(s) (“IAR”), whose signature(s) is/are below to perform the services under this Agreement. (Please note that IAR may also be a Registered Representative with Madison Avenue Securities, LLC, member FINRA/SIPC a registered securities broker‐dealer).
