This form is an agreement between Resultz Gym and the parent/caregiver of the child that wishes to gain membership. ALL 16 & 17 -year-olds must ensure that their parent/caregiver has filled out this form and they accompany you whilst you undertake your gym induction brief and issue you with your access card.
