THIS AGREEMENT, effective the date last executed below, by and between Public Utility District No. 1 of Cowlitz County, a Washington municipal corporation, hereinafter called the District, and a , hereinafter called the Contractor. The District and Contractor may be collectively referred to as the “Parties”, and individually as a “Party”. All services provided pursuant to this Agreement are subject to the terms and conditions set forth hereafter.
