This Addendum amends the HIE Participation Agreement (the “Participation Agreement”) between Chesapeake Regional Information System For Our Patients (“CRISP”) and (the “Program”), a facility that provides alcohol or drug abuse diagnosis, treatment or referral within the meaning of 42
