COMMONS REGISTRATION & USER AGREEMENTLiability Waiver • October 23rd, 2020
This Release of Liability must be signed by a legal guardian of any child under the age of 18 years of age who intends to use the facilities and/or equipment of Elkhart Health & Aquatics. This Release of Liability may remain on file with Elkhart Health & Aquatics to cover future visits by said child and will remain in effect until written notification is received by Elkhart Health & Aquatics revoking it.
