This Agreement is made and entered into by and between (parent as defined in Section 1000.21, Florida Statutes) on behalf of (hereafter collectively referred to as “Student”), and the District Board of Trustees of Pasco-Hernando State College (hereafter referred to as “PHSC”). The Student and PHSC hereafter shall collectively be referred to as the “Parties.”
